The graduate Physical Therapist shall work closely with his/her preceptor as well as with their Clinical Manager. The goal is to orient The graduate to become proficient in the delivery of Home Health physical therapy, to collaborate with his/her interdisciplinary team, the patient/family and assume the coordination of care.

  • It is the expectation The graduate will follow his/her Graduate orientation track as designed. This will ensure the observation and competency will be completed in the proper order
  • Performs clinical assessment to identify patient needs and eligibility
  • Document, prepare/review and complete a Plan of Care (POC), progress notes, phone orders and other clinical record documentation on an ongoing basis. Revise the POC based on evaluation, ongoing assessment data. Works in consultation with the physician and as part of an interdisciplinary team
  • lan and provide appropriate treatment based on medical referral and therapy assessments
  • Assess and reassess pain. Utilize appropriate pain management techniques. Educate the patient and family regarding pain management
  • Assist/instruct patient/family in transfer techniques appropriate to patient abilities. Instruct on use of equipment such as walkers, wheelchair, canes, braces, prosthetic devices and appliances in a safe manner. Utilize appropriate therapy equipment specific to patient diagnosis and needs. Orders appropriate rehabilitative equipment to meet patients' needs and adapts equipment, appliances to meet home limitations/requirements
  • Provides appropriate patient/family education regarding therapeutic procedures based on the patient's age, condition, culture and learning readiness. Applies concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease
  • Performs and oversees other team members, when applicable, in the performance of patient care that is consistent with patient's needs and desires. Will not supervise team members until completion of The graduate 90-120 day orientation. Must attend interdisciplinary team meetings/case conferences, staff meetings and other meetings as appropriate
  • Participate in Performance Improvement activities which may include clinical record review or participation in the PI Committee. Documents appropriately on all patient visits, case conferences, physician orders, re-certifications, discharges and care plan changes timely
  • It is the expectation the skills checklist will be completed with the preceptor when the patient care opportunity arises. Any skills not signed off by the preceptor, will be the responsibility of The graduate and his/her Clinical Manager to prove competency prior to delivery of that specific care
  • Accepts clinical assignments that are consistent with education and competence to care for patients
  • The following essential functions will not be assigned prior to the completion of the first 90-120 days. Supervise and provide clinical directions to the PTA to ensure quality and continuity of service provided. Provides case management under the supervision of the Clinical Manager in compliance with the state Practice Act. Weekend rotation/on-call without Preceptor

Required Skills:
  • Graduate of an accredited school of physical therapy
  • Current State License as an Occupational Therapist. Proof of current CPR certification.
  • Current driver's license. Must have reliable transportation and automobile insurance
  • If permitted by state regulations, the Graduate Occupational Therapist may be employed by the Home Health Agency when approved by the Regional Clinical and Operational Leadership. Prior medical experience is preferred

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
